A good Medicare FMO trains agents to protect it first.

Here’s the thing: Bexar County has one of the deepest public-sector retiree bases in Texas. You’ll meet retired teachers from Northside, North East, San Antonio, and Judson ISDs, state retirees under ERS, and retirees from the City of San Antonio, Bexar County, CPS Energy, SAWS, VIA, UTSA, and UT Health San Antonio. They live in Alamo Heights, Stone Oak, Helotes, Leon Valley, Converse, Live Oak, Schertz, Cibolo, the Southside and Westside, out toward Boerne, and up the New Braunfels corridor.

Many of them have group coverage they earned over a career. If the client isn’t sure, help them call their retirement system rather than guessing.

What happens if a TRS or ERS retiree enrolls in an individual Medicare Advantage or Part D plan?

For TRS retirees, it can be hard to undo. TRS says CMS rules prohibit mixing group and individual Medicare Advantage or Part D plans, and that enrolling in an individual plan means losing all TRS-Care coverage (TRS-Care Medicare Advantage FAQs). ERS handles it differently, but it still disrupts coverage.

TRS-Care

TRS bundles TRS-Care Medicare Advantage with TRS-Care Medicare Rx. Its FAQ says a member who opts out of TRS-Care Medicare Rx can’t join an individual drug plan while keeping the medical plan (TRS). Getting back in is hard. TRS says retirees and surviving spouses who leave generally can re-enter only at age 65 or after a qualifying special enrollment event, and that most participants past 65 never get another enrollment opportunity (TRS-Care Eligibility and Enrollment FAQs).

ERS

ERS says that if a HealthSelect Medicare Advantage member enrolls in an outside Medicare Advantage plan, Medicare removes them from HealthSelect MA and HealthSelect Medicare Rx. They keep group coverage through HealthSelect Secondary, where higher premiums may apply, and they have no group prescription coverage until they cancel the outside plan and re-enroll (ERS Medicare FAQs). Confirm with ERS how this applies to a specific client.

For local plans (City, County, CPS Energy, SAWS, VIA, university systems), get the plan’s written rules.

The takeaway: an individual enrollment for a group retiree isn’t a routine switch. If a client still wants to explore one, document why and make sure they’ve talked to their retirement system first.

Do public-sector retirees still need Medicare Part A and Part B?

Generally, yes. Group Medicare plans like TRS-Care Medicare Advantage and ERS HealthSelect Medicare Advantage build on Medicare, so clients usually need to stay enrolled in Medicare and keep paying the Part B premium. Check each plan’s exact requirement.

TRS says that starting January 1, 2026, Medicare-eligible TRS-Care participants must enroll in and pay for Part B to join and stay in a TRS-Care plan, and that not keeping Part B means losing all TRS-Care coverage (TRS turning 65 page). TRS-Care Medicare Advantage is for people with Parts A and B, or Part B only. ERS says HealthSelect Medicare Advantage requires enrollment in both Part A and Part B (ERS HealthSelect Medicare Advantage).

A client who drops Part B to save money can end up with a serious coverage problem. Point out the risk and send them to SSA and their retirement system.

On 2027 costs: ERS says premiums for Medicare-eligible retirees go up January 1, 2027. The state keeps paying 100% of health premiums for eligible retirees, but people who cover dependents or don’t get the full state contribution will pay more (ERS: Understanding Your 2027 Premiums). New Medicare retiree rates post on the ERS Fall Enrollment page in October. Send clients there.

Where does an independent agent actually add value for group retirees?

Your value is in the gaps around the group plan. That means a spouse who isn’t covered, a client who was never eligible or opted out, needs the group plan doesn’t address, and separate needs like life insurance or final expense.
