You can&#8217;t sell or change TRICARE or VA benefits \u2014 but you can coordinate around them, and the right FMO trains you to do it accurately.<\/span><\/p>\n<h2><b>Why is San Antonio different from every other Texas Medicare market?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Because of the military. San Antonio has been called &#8220;Military City USA&#8221; for decades, and that isn&#8217;t fluff \u2014 it changes the composition of your book.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> You&#8217;ve got Joint Base San Antonio spread across Lackland, Randolph, and Fort Sam Houston, plus Camp Bullis, and a retiree community that&#8217;s been settling here for generations. Bexar County carries an unusually high concentration of military retirees, veterans, and surviving spouses.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> Here&#8217;s the thing: in Houston or Dallas, a military-connected client is an occasional file you look up. In San Antonio, it&#8217;s a predictable part of your day. The coordination questions aren&#8217;t edge cases for you \u2014 they&#8217;re core competency.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> Then there&#8217;s the provider landscape: University Health, Methodist Healthcare, Baptist Health System, and Christus Santa Rosa on the civilian side, plus the Audie L. Your job is to understand the interactions in general terms, then send clients to TRICARE, the VA, or their own benefit administrators to confirm current rules for their situation.<\/span><\/p>\n<h2><b>What is TRICARE For Life and how does it work with Medicare?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">TRICARE For Life (TFL) is a Department of Defense benefit for Medicare-eligible military retirees and certain family members. It&#8217;s administered by Humana Military \u2014 not sold as a Medicare product, and not something you enroll anyone in.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> In general terms, TFL works as secondary coverage that wraps around Medicare. Medicare pays first on Medicare-covered services, and TFL then pays remaining eligible amounts under its own rules.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> The piece agents most need to remember: the client generally has to be enrolled in Medicare Part A <\/span><b>y<\/b><span style=\"font-weight: 400;\"> Part B to keep TFL eligibility. So if a 65-year-old retiree walks in thinking they can skip Part B because &#8220;I&#8217;ve got TRICARE,&#8221; that&#8217;s the conversation. You&#8217;re not selling anything in that moment \u2014 you&#8217;re keeping them from breaking their own coverage.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> And yes, a TFL client often doesn&#8217;t need much from you in the traditional sense. That&#8217;s fine. Being the agent who told them the truth is how you get their neighbor&#8217;s business five years from now.<\/span><\/p>\n<h2><b>Does a Medicare Advantage plan cancel TRICARE For Life?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">No, enrolling in a Medicare Advantage plan does not automatically cancel TRICARE For Life. But \u2014 and this is the part agents get wrong in both directions \u2014 it changes the coordination-of-benefits story significantly.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> With Original Medicare, coordination is relatively clean: Medicare pays, then TFL considers the remainder. When a client moves to an MA plan, the MA plan becomes primary inside its own network and rules structure. TFL&#8217;s coordination with an MA plan doesn&#8217;t behave the way it does with Original Medicare, and how claims settle can vary by situation.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> So please don&#8217;t say either of these:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">&#8220;MA cancels your TRICARE.&#8221; (Generally not true.)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">&#8220;Don&#8217;t worry, TRICARE will just pick up whatever the MA plan doesn&#8217;t.&#8221; (Also not something you should promise.)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">The accurate version sounds more like: &#8220;Your TFL doesn&#8217;t go away, but how the two coverages coordinate changes, and I want you to confirm the specifics with TRICARE before we do anything.&#8221; Then document that you said it. Your value here is helping them think it through \u2014 not steering.<\/span><\/p>\n<h2><b>Do TRICARE For Life beneficiaries need a separate Part D plan?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Usually not. TFL includes prescription drug coverage that&#8217;s generally treated as creditable, which means a TFL beneficiary typically doesn&#8217;t need a standalone Part D plan.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> &#8220;Creditable&#8221; matters because it&#8217;s what protects a client from a Part D late enrollment penalty later. So the honest answer is: you probably don&#8217;t need a PDP, here&#8217;s why, and here&#8217;s how to verify your drug coverage status directly with TRICARE.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> The 2027 backdrop is worth a glance. CMS&#8217;s July 28 release showed the standalone PDP market contracting from 709 plans down to 360, and the Part D Premium Stabilization Demonstration ends December 31, 2026. For most TFL clients that&#8217;s background noise \u2014 but know the landscape well enough to spot the client for whom it isn&#8217;t.<\/span><\/p>\n<h2><b>How does VA health care interact with Medicare?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">VA health care and Medicare do not coordinate benefits with each other. They&#8217;re separate systems, and that surprises a lot of clients.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> In general terms: care a veteran receives at a VA facility is paid through the VA. Care received outside the VA \u2014 with narrow exceptions like certain community care arrangements \u2014 is paid by Medicare or other coverage. There&#8217;s no cross-billing where one automatically covers the other&#8217;s gaps.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> This is why VA enrollment isn&#8217;t a substitute for Medicare enrollment. A veteran using the VA is still generally advised to enroll in Part A when eligible, and to think hard about Part B, because the VA doesn&#8217;t cover civilian care. If they get chest pain and end up at Methodist instead of Audie Murphy, the VA isn&#8217;t picking up that bill. Again, general framing only \u2014 VA eligibility tiers, copays, and community care rules are the VA&#8217;s to explain, not yours.<\/span><\/p>\n<h2><b>What about surviving spouses and CHAMPVA?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">CHAMPVA is a separate VA program for certain qualifying spouses, surviving spouses, and dependents of veterans, and it works differently from TRICARE.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> If you work San Antonio, you will meet surviving spouses, and some will be handling their own coverage for the first time in decades. The worst outcome is an agent who assumes &#8220;military family&#8221; means TRICARE and gives confidently wrong guidance.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> You don&#8217;t need to be a CHAMPVA expert. This is the single most common failure point with TFL clients.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Primary vs. secondary:<\/b><span style=\"font-weight: 400;\"> Walk through which coverage pays first in which setting \u2014 VA facility versus civilian provider, Original Medicare versus MA.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Provider loyalty:<\/b><span style=\"font-weight: 400;\"> Are they attached to a VA physician, a JBSA facility, or a civilian system like University Health or Christus?<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Stated priorities, documented:<\/b><span style=\"font-weight: 400;\"> Cost, provider access, simplicity \u2014 write down what they said mattered.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Scope honesty:<\/b><span style=\"font-weight: 400;\"> Say out loud that TRICARE and VA rules must be verified with those agencies, and note that you said it.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Bilingual capacity matters here too. Spanish-speaking demand across Bexar County is strong, and plenty of military-connected households are bilingual. If you can run this checklist in both languages, you stand out.<\/span><\/p>\n<h2><b>What 2027 changes should you plan around with military-retiree clients?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">The MA carrier exit wave is expanding for 2027, and some military retirees on MA plans will be forced to move. That&#8217;s your calendar problem.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> Reported pullbacks include Humana (roughly 600,000 members), UnitedHealthcare (34 counties, 20,000-plus members), Molina exiting MA entirely, Providence winding down, and Presbyterian pulling back \u2014 up to around 3 million seniors nationally. A non-renewal conversation with a TFL client is more delicate than a standard switch, because the coordination question resurfaces.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\"> Two other items: CMS CY2027 marketing rules take effect October 1, 2026 \u2014 the 48-hour Scope of Appointment hold is gone and TPMO disclaimer timing changes.
